Adult Family Care

Through adult family care, people become caregivers for family members with qualified medical diagnoses⸺including intellectual, developmental, or physical disabilities⸺that results in the member needing assistance and oversight to safely manage specific needs, while living at home.

Adult family care helps support a safe, nurturing home for people who need assistance to complete the activities of daily living, such as dressing, bathing, eating nutritious meals, walking, and mobility. With information from their health care providers, we work with people and their identified care team to create a person-centered Plan of Care.

Monthly visits from an RN, care manager, and community health worker offer detailed health service coordination, communication, and related support. This dynamic service delivery plan allows for AFC member engagement in the planning and receiving of care in the comfort of their own home.

Nonotuck Resource Associates, Inc. facilitates services and resources such as nursing, case management, and clinical care for the member and financial support via a biweekly stipend for the family caregivers.

People who need help are able to stay with their families in the familiar comfort and stability of their own homes and communities.
